PHONE CONSULTATION

Our phone consultations have proven to be very effective next to an actual hands-on exam. Please follow the procedures listed below so your cost will be as minimum as possible. Our desires are to help people, not to get rich quick.

Phone Consultation Procedures:

1. Fill out the Consultation Sheet. (This form tells us your history and guides the practitioner to reasons for your call. Any and all of this information is voluntary and will remain in strictest confidence within the Institute.)

2. Fill out the Client Questionaire. (This form has been created by the Institute based on its teachings and practices. Some of the questions are duplicated and should be answered each time if applicable.)

3. Write a detailed account of your symptoms. Include such things as your feelings, changes in your normal environment (work and home), and any strange or peculiar sensations.

4. Fax the three forms above to (202) 966-8999 with a cover letter to the Washington Institute.

5. Call the Institute at (202) 237-7681 to setup a time for your phone consultation. This will be a future time when you will call the Institute and give credit card information for payment. (Checks with one week grace period, money orders, or other forms of payment may be considered.)

6. It is recommended that two weeks after you start the treatment, you make a call to schedule a free follow-up with the practitioner. (Some treatments work right away, some take time, and still others may uncover other, previously hidden issues.)
